How to Register for PhilHealth in the Philippines: A Guide for New Employees

PhilHealth is the Philippine Health Insurance Corporation โ€” providing government health insurance coverage for all Filipinos. As a formal employee at ShoreAgents (or any registered employer), PhilHealth contributions are mandatory and beneficial.

Here's how to register if you don't yet have a PhilHealth number.

What PhilHealth Covers

Your PhilHealth membership provides coverage for:

  • Inpatient hospital admissions
  • Maternity packages
  • Z-packages for catastrophic illnesses (cancer, kidney disease, etc.)
  • Selected outpatient procedures
  • Advance payment of benefits at accredited hospitals

Coverage amount depends on your member status and the specific medical procedure.

How to Register: Online

PhilHealth has an online registration system at eregister.philhealth.gov.ph

Steps:

  1. Visit the PhilHealth online registration portal
  2. Select "Individual" or "Employee" registration
  3. Fill in your personal details:
    • Complete name
    • Date of birth
    • Sex
    • Address
    • Contact information
  4. Submit the form
  5. You'll receive a PhilHealth Identification Number (PIN) online

Note: You may still need to submit physical requirements to a PhilHealth office to complete full registration and receive your PHN (PhilHealth Identification Number) card.

How to Register: Walk-In

Required Documents:

  • Completed PMRF (PhilHealth Member Registration Form) โ€” available at any PhilHealth office
  • Original and photocopy of any valid ID (birth certificate, passport, driver's license)

Steps:

  1. Visit the nearest PhilHealth Local Health Insurance Office (LHIO) or branch
  2. Submit completed PMRF with supporting documents
  3. Receive your PhilHealth Number and Member Data Record (MDR)

Member Data Record (MDR)

Your MDR is the key document your employer needs to register you as an employee-member and begin contributions. Print this and provide it to our HR team.

Your MDR shows:

  • Your PhilHealth Identification Number
  • Your personal information
  • Listed dependents (spouse and children can be listed as dependents)

Listing Dependents

As a PhilHealth member, you can list qualified dependents:

  • Legal spouse
  • Unmarried children under 21 years old
  • Dependent parents (if you have no listed spouse)

Dependents receive coverage under your membership.

After Registration

Once you provide your PhilHealth number to ShoreAgents HR, we register you as an employee-member and begin the employer-employee contribution system. Contributions are deducted from your salary and matched by the employer as required by law.

  • View contribution history
  • Print MDR
  • Check benefit eligibility
  • Update personal information
